How to register for the review
- Step 1: Download and print a copy of the registration form. If you don't have a printer, you may request for a copy to be mailed to you by completing the request form below or by calling (773)545-8425 or (630)483-7244.
- Step 2: Select the the review schedule you want to sign up for and complete the registration form.
- Step 3: Make your payment (cashier's check or money order) payable to Excell Professional Services, Inc. WE DO NOT ACCEPT PERSONAL CHECKS. Please contact us for other payment options.
- Step 4: Mail the completed form and your payment to:

Registration forms must be received ten (10) days before the start of the class. Please call us to confirm receipt of your payment and registration.
